Streamline Your Patient Workflows with Formstack Sign and SMARTcare

If you’re using SMARTcare’s home care platform to connect with and monitor your patients, we have a way to improve efficiency for your back office. Formstack’s HIPAA compliant eSignature solution, Formstack Sign, easily integrates with SMARTcare so you can securely collect and sign critical patient documents. 

With Formstack Sign and SMARTcare, you can:

  • Quickly gather patient signatures
  • Automatically store completed documents in a single repository
  • Reduce security risks with products that exceed compliance standards

The Formstack Sign platform is built to ensure that anyone in your organization can build the workflows they need without code. Let’s take a look at how easy it is to set up your SMARTcare and Formstack Sign integration. 

Head over to your Formstack Sign account and choose Upload File.

Next, add your participants. Here, you can add as many participants as you need and choose to send the eSignature request by email or SMS text message. 

When you’re done adding participants, choose next. Formstack Sign offers a variety of field options to help you capture the information you need from your patients, clients, and employees. 

For example, our document includes a series of questions where the signer will need to select an answer. For these questions, we’ll use the Radio button field. Similarly, we’ll add a text box to the bottom of the questionnaire to capture any additional information. Here’s what our document looks like after we’ve added our question fields. 

Now, we’re ready to add our eSignature fields. We’re using the name field where we’d like the signers full name to be printed and using the eSignature and date fields where necessary. We’ll do the same for our second signer. Here’s what our document looks like after all of our fields have been added. 

Once our tags have been added, we’re ready to add a message for our signer. Here, you can choose to password protect your document, carbon copy additional stakeholders, or set up reminders to be sent daily or weekly. 

Now that your document is set up, you’re ready to integrate with your SMARTcare account. Setting up your integration couldn't be easier! Just reach out to the support team at SMARTcare using this link and request that they connect your Formstack Sign account with your SMARTcare account.
