1. I consent to my image, likeness, voice, personality, and name (collectively, “Likeness”) being photographed, recorded, and/or filmed by the Jets or any of their assigns, successors, parent, affiliated and subsidiary companies (collectively the “Jets Entities”) at the Shoot.

2. I grant the Jets and the Jets Entities and each of them the irrevocable right to make photographs, video and/or sound recordings of my appearance in the Shoot, and to use, and reuse, the resulting photographs, films, tapes, video and sound recordings (collectively the “Images”), as well as my Likeness and the Testimonial Quote, in the production, distribution, exhibition, publishing, advertising, promotion, marketing, and publicity materials of the Jets throughout the universe, in perpetuity, in any and all media, manners and formats, now known or hereafter devised. The Shoot or any part thereof (including without limitation my Likeness, the Testimonial Quote, and the Images) can be edited, cut, added to, adapted, dubbed, modified and/or rearranged at the Jets’ sole and absolute discretion. I hereby waive unconditionally and irrevocably the benefit of any provision of law known as “moral rights” or similar laws now or hereafter prevailing in any part of the world.

3. The Images and all contents thereof shall be owned entirely by the Jets, and my contribution to the Images is specially ordered by the Jets as a work made for hire for the Jets. The Jets are not obligated to use my Likeness, the Testimonial Quote, or the Images in any way whatsoever.

4. I understand and acknowledge that I shall not receive any monetary compensation for participation in, or otherwise in connection with, the Shoot, or for the exercise of any other rights granted by me herein.

5. I and my representatives, heirs, successors and assigns, hereby absolutely, unconditionally and forever release and discharge the Jets and the Jets Entities, all other persons and entities connected with the Shoot, and each of their respective present, future and former members, officers, directors, agents, representatives, attorneys, employees, successors, assignees, and licensees, (collectively the “Released Parties”) from any and all claims, actions, causes of action, proceedings, suits, awards, judgments, damages, liabilities, losses, costs and expenses of any kind (including, without limitation, reasonable attorneys’ fees) arising out of, resulting from, or by reason of, the use of my Likeness and the Images and my participation in or in connection with the Shoot (including without limitation personal injury, emotional distress, invasion of rights of privacy and/or publicity, and/or defamation).

6. I acknowledge that I have read this Liability and Publicity Release prior to its execution and that I fully understand its contents. This release constitutes the entire agreement and understanding between me and the Jets concerning the subject matter hereof, and supersedes and replaces all prior negotiations, proposed agreements and agreements, written and oral, relating thereto. I understand that this release cannot be changed or terminated except by a written instrument signed by the Jets and me. This release and the rights hereunder may be freely assigned by the Jets. This release shall be governed by the laws of the State of New Jersey without regard to its conflicts of law rules.
