VOLUNTEER AGREEMENT AND RELEASE OF LIABILITY

I wish to be a volunteer at Muncie Civic Theatre, an Indiana not-for-profit corporation located at 216 E. Main Street, Muncie, IN 47305 and at any other locations at which Muncie Civic Theatre is using volunteers or performing. I certify that the information in this Agreement and Release of Liability, as entered by me, is truthful.

I acknowledge that while volunteering, I may be involved in activities which involve a certain amount of possibility that I may be injured and I assume that risk of injury and relieve Muncie Civic Theatre of any claim against it caused by injury which may occur to me while I am volunteering.

On behalf of myself, my heirs, assignees and personal representatives, I do release and forever discharge Muncie Civic Theatre, Inc., its employees, agents, members, sponsors, promoters, and affiliates from any and all liability, claim, loss, cost or expense, and waive and promise not to sue on any such claim against any such person or organization arising directly or indirectly from or attributable in any legal way to any negligence, action or omission to act of any such person or organization, including travel to and from such event, in which I may participate as a volunteer.

I currently have no known physical or mental condition that would impair my capability to volunteer at Muncie Civic Theatre.

I, as parent or guardian of the above applicant, represent to Muncie Civic Theatre, Inc. that the facts in this document concerning my child or ward are true. I give my permission for my child or ward to perform volunteer work at Muncie Civic Theatre as set forth hereinabove for, and further, in consideration of the granting of such opportunity to volunteer and participate in the activities of Muncie Civic Theatre I agree individually and on behalf of my child or ward, to the terms of the above Agreement and Release of Liability.

Actor's Code of Conduct

CAST EXPECTATIONS & CONTRACT

  • Please be punctual to every call. If you are running late, please call/text the stage manager immediately.
  • All cast members are expected to wear comfortable clothing that allows you to move during rehearsals. You must always wear closed-toed, rubber-soled shoes to every rehearsal.
  • Please bring a pencil, your script, and a recording device to music rehearsals (ie. cell phone). 
  • Please do not cut, dye, or alter your hair without speaking to our hair designer first. 
  • Please limit any public media posts regarding the show to sharing those through the official Muncie Civic pages.
  • There is also a mandatory strike to follow the final performance. 
  • Do not eat or drink in the house of the theatre (the new auditorium seating). Please do so in the lobby, rehearsal hall, or kitchen. Throw away any trash that you see. 
  • If you see or experience something that is not up to Muncie Civic standards regarding behavior, environment or safety please take responsibility and talk to a staff member.
  • Each cast and crew member receives one free complimentary ticket.
  • Muncie Civic Theatre reserves the right to immediately remove from any production, at any time, any actor whose behavior proves to be detrimental to the safety and well-being of another performer, staff member, or volunteer involved with the creative process. Such behavior includes, but is not limited to, showing up to the theatre intoxicated or otherwise under the influence of a controlled substance; physically or verbally threatening or harming another individual, or willfully causing physical damage to the theatre itself. In addition, any behavior in the community which calls into question the character of any volunteer and which, in the sole determination of the Executive Committee of the Board of Directors, would reflect poorly on the theatre if that volunteer were to continue their participation in any program or production, is grounds for the volunteer’s participation to be terminated.
  • I hereby grant Muncie Civic Theatre permission to videotape and photograph me or my child during the production process. I give permission for Muncie Civic Theatre to use any video footage and photographs taken of me or my child for newspapers, educational settings, professional conferences, other media advertisements, and Muncie Civic Theatre’s website.
