I understand that any false information or omission may disqualify me from further consideration for employment, or, if I am hired, may result in my dismissal from employment if discovered at a later date.
I expressly authorize, without reservation, Bellevue Baptist Church, its representatives, employees or agents to contact and obtain information from all references (personal and professional), employers, public agencies, licensing authorities and educational institutions and to otherwise verify the accuracy of all information provided by me in this application, resume, or job interview. I hereby waive any and all rights and claims I may have regarding the employer, its agents, employees or representatives, for seeking, gathering and using such information in the employment process and all other persons, corporations or organizations for furnishing such information about me.
I understand that if offered a position with Bellevue Baptist Church, I may be required to submit to a pre-employment medical examination, behavioral and/or psychological assessments, drug screening and background check as a condition of employment. I understand that unsatisfactory results from, refusal to cooperate with, or any attempt to affect the results of these pre-employment tests and checks will result in withdrawal of any employment offer or termination of employment if already employed.
If employed, I agree to engage in no outside activity which would involve a conflict of interest with, or which could reflect adversely on Bellevue Baptist Church. I understand this decision is to rest with church leadership.
If employed, I agree to hold in strictest confidence any information concerning the church, its members, and its employees which may come to my knowledge.
I understand that this application remains current for only 30 days. At the end of that time, if I have not heard from the church and still wish to be considered for employment, it will be necessary to reapply and fill out a new application.
It is my understanding that Bellevue Baptist Church is an at-will employer and by understanding this it has been explained to me that if I am hired, I understand that I am free to resign at any time, with or without cause and without prior notice, and the church reserves the same right to terminate my employment at any time, with or without cause and without prior notice, except as may be required by law. This application does not constitute an agreement or contract for employment for any specified period or definite duration. I understand that no supervisor or representative of the employer is authorized to make any assurances to the contrary and that no implied oral or written agreements contrary to the foregoing express language are valid unless they are in writing and signed by myself and the Administrative Pastor of Bellevue Baptist Church. I understand that in addition to the employment application process, any consideration of employment will depend on a satisfactory completion and approval of the Confidential Safety Application and any other conditions of employment that may apply.
I understand and agree that it is critical to the mission and ministry of Bellevue Baptist Church that all employees confirm to the highest standard of safety, interpersonal conduct, and sexual morality. I affirm that I will strictly comply with Bellevue Baptist Church’s policies and procedures, including those concerning child safety and protection, sexual abuse and misconduct, and interpersonal relationships. I understand and agree that failure by me on abide by such policies and procedures may result in my immediate dismissal.
I also understand that if I am hired, I will be required to provide original documents as proof of identity and legal authority to work in the United States and that federal immigration laws require me to complete an I-9 Form in this regard. I agree to read, understand, agree with, and abide by the Statement of Faith.
I agree that if I become employed by Bellevue Baptist Church, I hereby give Bellevue Baptist Church authorization to release the following information regarding my employment to potential employers who call for references and/or employment verification: dates of employment, position, attendance, salary, performance, and eligibility for rehire.
I agree that if Bellevue Baptist Church employs me, in the future a potential employer may contact Bellevue Baptist Church or its representatives concerning my work record and my performance at Bellevue Baptist Church.
I hereby consent to and authorize persons employed by Bellevue Baptist Church to divulge any and all information they consider relevant to any person representing him or herself to be an employer or potential employer of mine with respect to my work and/or performance of my job at Bellevue Baptist Church. This consent specifically includes any information related to any allegation or investigation of child abuse or sexual abuse or molestation in any form. If it comes to the attention of Bellevue Baptist Church that I am being considered for employment or I am employed by any organization involving children and I have been terminated or suspended for sexual misconduct, I understand that Bellevue Baptist Church can voluntarily contact the organization to share this information. I further release and hold harmless Bellevue Baptist Church, supervisors, and co-workers from any and all liability that may potentially result from the release and/or use of such information to any employer or potential employer who calls for a reference and/or employment verification.
